Driver in fatal Salem crash is Pennsylvania State Police trooper from Uniontown

A Pennsylvania state trooper remains on the job after his involvement in a fatal Aug. 17 crash in Salem, officials said Monday.

A BMW driven by Tyler J. Strini, 27, of Homer City did not slow down for stopped traffic, troopers said.

Strini, assigned to Troop B in Uniontown, joined the state police in 2020.

John Farally, 56, of Pitcairn was waiting at a stoplight on Route 66 north at 6:30 a.m. Aug. 17, when the Mazda 3 he was driving was hit from behind. He died after being taken to a hospital.

State police communications director Myles Snyder said Strini was off duty and headed home after a shift at the time of the crash.

He remains on active duty, and state police are continuing to lead the investigation.

Snyder said that while state police cooperate with other investigative agencies in use-of-force incidents such as a trooper-involved shooting or a pursuit ending in a fatal crash, “in a case like this, where a member is involved in an off-duty crash, we are confident that highly trained investigators, such as our collision analysis reconstruction specialists — who receive hundreds of hours of training to reconstruct and document crash scenes — can properly document and reconstruct crash scenes.”

“Of course, the district attorney will be kept apprised of the evidentiary findings, and the investigation will proceed accordingly,” Snyder said.

Farally was taken to AHN Forbes Hospital in Monroeville after firefighters extricated him from the car, according to police.

He died at the hospital at 10:30 a.m. Aug. 18, according to the medical examiner.

Strini refused medical transport to a hospital but went to a UPMC hospital to be evaluated for minor injuries, according to the state police crash report. No charges related to the crash have been filed, according to online public court records.
