Excela Health of Greensburg will conduct a covid vaccination clinic for Penn-Trafford students age 12 and older as part of its fall athletic physicals at the Penn-Trafford High School in Harrison City.
The clinic will take place June 3 from 8 a.m. to noon.
The clinic also is open to adults who are accompanying a student.
The second shot will be administered on June 24 at the same location, Excela Health said.
Excela Health requires recipients to register because of requirements for storing the vaccine.
Those who register will receive an email with their appointment time.
Students registering for fall sports can use a link to register for their sport, physical, and/or vaccine: https://www.familyid.com/programs/2021-2022-fall-sports-athletic-physical-registration.
For those seeking to have their student or themselves get the vaccine, register using the following link: https://docs.google.com/forms/d/e/1FAIpQLSfy4NQv7DhJGAOwjwM1EylakEURnt1tEvfb-QMGcKgOf5GArw/viewform.
A “Vaccine Authorization Form – Pediatric” must be completed by those receiving a vaccine. Those younger than 18 must have an authorization form completed by a parent or guardian.
Anyone being immunized should show a state-issued ID, if available, and a copy of their health insurance card. If uninsured, vaccines will be provided free of charge.
